Job Description:
LanceSoft is seeking a travel nurse RN Home Health for a travel
nursing job in Ronan, Montana. Job Description & Requirements
RN - Case Manager provides skilled nursing, care, and case
management for
assigned patients in their place of residence; admits patients onto
services;
coordinates and integrates services to ensure continuity of care;
facilitates
the achievement of quality, clinical and cost outcomes; negotiates,
procures
and coordinates services and resources needed by the patient or
family; creates
opportunities and systems to enhance outcomes; and uses the nursing
process of
assessment, planning and implementation of care, including
patient/family
instruction and evaluation through an inter-disciplinary team
approach.
This
position may start as a Visiting RN and move to Case Manager. The
same clinical
responsibilities apply to both designations. To move to Case
Manager, the
incumbent must carry a half case load as described in the
Collective Bargaining
Agreement with the Montana Nurses Association Local #32. About
