Friday Hours per Shift: 8:00 AM - 5:00 PM with Call Experience:
Minimum 2 years of recent acute care, home care, or hospice
experience License: Current RN license in the State of Montana (BSN
preferred) Certifications: Valid driver’s license, appropriate auto
insurance, and reliable transportation Must-Have: Clinical skills
in venipuncture, catheterization, wound care, and/or parenteral
therapy Description: The RN Case Manager provides skilled nursing,
care, and case management for assigned patients in their place of
residence. The RN admits patients onto services and coordinates and
integrates services to ensure continuity of care. This role
facilitates the achievement of quality, clinical, and cost
outcomes. The RN negotiates, procures, and coordinates services and
resources needed by the patient or family. Opportunities and
systems are created to enhance outcomes. The nursing process of
assessment, planning, and implementation of care is utilized,
including patient/family instruction and evaluation through an
interdisciplinary team approach. This position may start as a
Visiting RN before moving to Case Manager. The same clinical
responsibilities apply to both designations. The RN provides
skilled nursing and coordinates care to patients and family members
in a residential setting through a team approach. Care is based
upon systematic evaluation as authorized by a physician. The RN
observes, assesses, evaluates, and plans interventions in relation
to patients' physical and psychosocial statuses, including changes
in condition and reactions to treatments or medications. Oversight
is provided for care given by Home Health Aides and CNAs to
assigned patients, evaluating compliance with the plan of care.
Continuity of care is assured through effective communication,
information exchange, and case conferences with other disciplines,
community health, support, and social agencies. Quality care and
appropriate symptom management are provided to promote optimal
health and quality of life through teaching, counseling,
appropriate interventions, and referrals. The RN maintains
accurate, current, and complete patient records in accordance with
agency standards and knowledge of current reimbursement rules and
regulations. Participation in team scheduling coordination and
on-call rotation schedule is necessary to meet patient care needs
by assessing and evaluating patient needs via telephone and
advising or making home visits as required. Participation in
quality improvement activities and attendance in-service training
is expected. Other duties may be assigned by the supervisor.
