Job Description:
Description The Nurse Case Manager (NCM) is responsible to
manage a caseload of patients across the continuum of healthcare
services and assure optimal quality outcomes, patient satisfaction,
and appropriate use of resources. The NCM is responsible for
directing an organized, patient-centered interdisciplinary team
system that coordinates the delivery of healthcare services by
continually monitoring, evaluating, and modifying the treatment
plan to achieve optimal patient outcomes and efficacious use of

Status and Shifts Available: - Per Diem / Day. Required
Qualifications: - Graduate of an accredited school of nursing. -
Montana Registered Nurse License upon hire. - 3 years - Experience
in Clinical Nursing. - 1 year - Hospital case management
experience. - Must live within the Providence footprint that is
also part of the Central Division - Montana (Missoula or Polson) -
Eastern Washington (Spokane, Walla Walla, or Richland) - Texas
(Lubbock) - New Mexico (Hobbs) - Oregon (Portland, Medford,
Beaverton, Newberg, Canby, Milwaukie, Tigard, Seaside, or Gresham)
Preferred Qualifications: - Current Case Manager Certification upon
