Care Manager RN - Assisted Living at Providence Health and
Services summary: The Nurse Case Manager (RN) in Assisted Living
coordinates comprehensive patient care by managing caseloads,
leading interdisciplinary teams, and optimizing treatment plans to
ensure quality outcomes and resource efficiency. The role involves
collaboration with physicians and social workers to deliver
cost-effective, evidence-based healthcare services within a
multidisciplinary system. This position requires nursing licensure,
experience in healthcare, and focuses on supporting vulnerable
populations in a patient-centered environment. Description The
Nurse Case Manager (NCM) is responsible to manage a caseload of
patients across the continuum of healthcare services and assure
optimal quality outcomes, patient satisfaction, and appropriate use
of resources. The NCM is responsible for directing an organized,
patient-centered interdisciplinary team system that coordinates the
delivery of healthcare services by continually monitoring,
evaluating, and modifying the treatment plan to achieve optimal
patient outcomes and efficacious use of resources. The NCM
coordinates with physicians, Social Worker, and other health team
members to expedite medically appropriate, cost-effective care and
promote quality through evidence based medical practice throughout

Qualifications: Graduate of an accredited school of nursing. Upon
hire: Montana Registered Nurse License. 1 year Experience in
nursing/related field. Preferred Qualifications: 2 years Experience
