Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Description American Traveler is seeking a Physical Therapist for a home health assignment, requiring at least 1 year of recent skilled home health experience and BCLS certification. Responsibilities - Position is based in home health settings serving Salem, OR and surrounding areas including South Salem, West Salem, Corvallis, Albany, Sweet Home, and Lebanon - Day shift schedule with 5x8 hour shifts, 8:00 AM to 4:30 PM - Caseload includes up to 6 patients per day with 1:1 patient visits and 30 productivity points standard - No weekend, on-call, or floating to like units required, but may be expected to float to other facilities within the state - Epic EMR and Oasis documentation required, with use of Epic Remote Client (laptop) and Rover (phone app) - Travel radius may occasionally extend up to approximately 60 miles or 60–90 minutes of drive time daily - Business casual attire is required; scrubs are permitted but not required with no specific color - Responsibilities include patient assessment, developing individualized care plans, and providing skilled therapy services in patients' homes - Mileage is reimbursed at the IRS standard rate - All travelers must be willing to float between facilities within the state if required - Modules and online training are completed pre-start and annually, averaging 16 hours - First-time travelers are welcome to apply - Former permanent staff must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the facility - Minimal to no RTO is preferred; extended or multiple days off may impact assignment consideration - Free parking is available at all work sites Requirements - Active Physical Therapist license required if applicable; pending license may be accepted - Minimum of 1 year of recent skilled home health experience - Current BCLS certification required - Demonstrated proficiency with Oasis documentation required - Epic EMR experience required - Reliable personal transportation is required - Completion of primary COVID vaccine series plus at least one booster is required - Must provide two supervisory references from within the last three years in a comparable setting Benefits - High Pay and Bonuses - Medical, Dental and Vision Insurance with Day 1 Options - 401(k) Plan - Weekly Payroll Deposit - Free Online CEUs - Generous Housing Allowance - Travel and Licensure Reimbursements - Non-taxed Per Diem and Subsidy - Traveler Rewards and Discounts American Traveler Job ID P-641001.
