ROLE OVERVIEW: The Licensed Practical Nurse (LPN) is essential to
our team’s success, contributing to a welcoming environment that
residents can proudly call home. The main objective of this role is
to deliver high-quality, holistic nursing care in line with
established nursing standards and healthcare regulations. You will
lead and guide both licensed and non-licensed staff in providing

Positions SHIFT DETAILS: Full-time, Day Shifts: 12-hour shifts from
6:30 AM to 7 PM Availability for rotational weekends and holidays

length of service, and/or location. Key Responsibilities: Perform
daily nursing duties in compliance with current regulations and
guidelines for long-term care facilities. Ensure nursing staff
adheres to established policies and safety protocols. Manage the
admission, transfer, and discharge processes for residents,
documenting care and services provided. Assist in orienting new
residents and their families to the facility. Conduct rounds with
healthcare providers, hospice staff, and home health personnel as
necessary. Utilize nursing staff according to residents' care plans
and healthcare provider orders. Communication and Documentation
Duties: Conduct daily clinical and environmental assessments to
monitor resident care and report significant occurrences. Address
and resolve issues with residents and/or their families, escalating
significant matters to management. Participate in facility
inspections as requested by management. May oversee clinical
programs as assigned, such as dementia care, wound management,
quality assurance, and infection control. Complete initial
assessments and assist in developing interim care/service plans.
Update and review care plans to reflect goals, issues, approaches,
and modifications based on nursing needs and healthcare provider
orders. Collaborate with healthcare providers to review treatment
plans and ensure compliance with established policies. Review
residents' medical records for treatment, medication orders, diets,
and lab results to ensure proper implementation. Request and
coordinate diagnostic and therapeutic services as ordered by
healthcare providers. Communicate departmental policies to staff,
residents, and visitors. Provide written and verbal reports
regarding changes in resident conditions and other shift activities
to nursing management. Complete accident/incident reports involving
residents and submit them to the Director of Nursing. Document all
medical records accurately to support care services. Provide
information for care conferences or other meetings involving
residents and families. Report discrepancies related to physician
orders, dietary changes, or charting errors to the supervisor.
Ensure clinically accurate documentation in accordance with policy.
Review interdisciplinary team progress notes to verify adherence to
care/service plans and report concerns to your supervisor. Inform
nursing staff of new admissions and their care requirements,
ensuring readiness of rooms for new residents. Notify healthcare
practitioners and families of any changes in resident conditions.
Assist in arranging transportation for residents and coordinating
third-party services. Medication Administration Responsibilities:
Prepare and administer medications as prescribed by healthcare
providers. Coordinate with pharmacies to ensure availability of
resident medications. Maintain adequate supplies of medications and
nursing equipment, reporting needs to the supervisor. Review
medication administration records for accuracy and completeness,
addressing concerns with your supervisor. Dispose of medications
and narcotics according to established procedures. Personnel
Management Duties: Oversee nursing unit operations to ensure staff
compliance with nursing standards. Collaborate with the Director of
Nursing to address staff performance issues, including potential
terminations. Report complaints and grievances to management and
work with the Director of Nursing to develop action plans. Safety
and Sanitation Responsibilities: Ensure availability of protective
equipment for staff. Maintain cleanliness and sanitation in nursing
service areas, resident rooms, and treatment spaces. Recommend
equipment and supply needs to the Director of Nursing, ensuring
adequate stock levels are maintained. Perform other job-related
tasks as assigned. Knowledge and Essential Skills: Ability to make
independent decisions and follow instructions effectively. Skill in
interacting tactfully with personnel, residents, families,
visitors, and government representatives. Capability to work with
individuals who are ill, disabled, elderly, or emotionally
distressed. Effective communication skills for interaction with
supervisors, team members, residents, and families. Education and
Experience Requirements: Must fulfill all relevant state and
federal criteria for this position. Must hold a current, active,
and unencumbered LPN license in the applicable state, maintaining
good standing throughout employment. Preferred: One year of nursing
experience in home care, assisted living, or long-term care
settings. Supervisory Responsibilities: This role requires the LPN
to ensure that resident care is delivered according to established
standards, with CNAs/Caregivers providing support without direct
