a qualified Home Health RN in Missoula, Montana, 59801! Job Title:
RN Case Manager Profession: Nursing Specialty: Hospice RN, Home
Health RN Duration: 13 weeks Shift: Monday to Friday Hours per
Shift: 8:00 AM to 5:00 PM with on-call duties Experience: Minimum 2
years License: Current RN license in the State of Montana (BSN
preferred) Certifications: Valid driver’s license, appropriate auto
insurance, and reliable transportation Must-Have: - 2 years of
recent acute care, home care, or hospice experience - Clinical
skills such as venipuncture, catheterization, wound care, and/or
parenteral therapy Description: The RN Case Manager provides
skilled nursing care and case management for assigned patients in
their place of residence. This role involves admitting patients
onto services and coordinating care to ensure continuity. The RN
facilitates the achievement of quality, clinical, and cost
outcomes. Negotiating and procuring services and resources needed
by the patient or family is essential. The RN creates opportunities
and systems to enhance patient outcomes. Utilizing the nursing
process of assessment, planning, and implementation of care is
crucial. The RN provides patient and family instruction and
evaluates outcomes through an inter-disciplinary team approach. As
part of the role, skilled nursing and care coordination are
provided based on systematic evaluations authorized by a physician.
The RN observes, assesses, and evaluates patient conditions,
planning interventions as necessary. Overseeing care provided by
Home Health Aides and CNAs is part of the responsibilities. The RN
assures continuity of care through effective communication and case
conferences. Quality care and appropriate symptom management are
provided to promote health and quality of life. Maintaining
accurate and complete patient records in accordance with required
standards is necessary. The RN also participates in scheduling
coordination and on-call rotation as required to meet patient care
needs. Contributing to quality improvement activities and attending
in-service training are essential duties. The RN performs other
