Job Description:

This is a full time (0.8 - 1.0 Status), day shift Emergency Department Care Navigator position in the Emergency department.
The Care Navigator (CN) will be stationed in the Emergency Department and work in collaboration with the Emergency Department team, Provider staff, and others as needed. The CN will provide customer service-oriented and patient-centric attitude, effective planning and organizational skills, the ability to be accountable for quality customer service and respond appropriately to a variety of personalities and situations, and ability to maintain patient confidentiality.
ESSENTIAL FUNCTIONS:
To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation.
--- Directly assist patients with scheduling appropriate follow-up visits before their discharge from the emergency department.--- Use low volume periods to contact patients who presented between CN coverage.--- Educate patients on the importance of seeking follow-up care and being engaged in their care journey.--- Record basic data regarding each patient interaction (e.g., whether the patient was able to schedule an appropriate follow-up visit).--- Help ensure patient follow-up by providing patient reminders (e.g., text, phone call, etc.).--- Support hospital strategy for the hospital.--- Maintain regular and reliable attendance.--- Perform other duties as assigned.
Additional Information:The position serves both internal co-workers and external customers, clients, patients, contractors, and vendors.Access to and/or works with sensitive and/or confidential information.Exhibit a comprehensive understanding of healthcare regulatory and compliance (e.g., HIPAA). Skilled in the application of policies and procedures. Knowledge of Business Office Standards and Recommended Practices.
Skills and Abilities:Mathematical Skills--- Business Mathematical Skills -- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Computer Skills.--- Advanced Computer Skills -- Routine use of electronic mail and computer systems. Ability to incorporate complex and complicated functions into documents, spreadsheets, databases, and presentations to support business objectives.Communication--- Complex Communication -- Frequently communicates complex information and interacts with management. Can present, resolve and address delicate situations. Can motivate and persuade others.Nature of Problems--- Varied Business Problems -- Problems are varied and complex, requiring analysis or interpretation of the situation. Problems are solved using knowledge and skills, general precedents and practices.Independent Judgment--- Foundational Judgment -- Results are defined, and existing practices are used as guidelines for how to complete work activities; works closely with supervisor/manager who provides broad guidance and overall direction.
EDUCATION AND EXPERIENCE


  • High school or GED equivalent required.
  • College degree preferred.
  • Minimum of 2-3 years customer-facing and/or clerical experience, preferably in a clinic, physician's office, or other medical setting.
  • Medical clerical experience and medical terminology preferred.
  • Proficiency in Microsoft Office applications, health care patient records/scheduling/billing/order systems, and/or an electronic health record system preferred.
