$26.00) Mountain Home is seeking an Adult Case Manager (CM) to
enhance the outcomes, experiences, and overall value for the
families we serve. The CM will provide comprehensive case
management services to young mothers, adolescents, and children,
connecting them to essential medical, social, educational,
vocational, and recreational services. Additionally, the CM will
serve as an advocate, supporting families in building their
independence and increasing their self-sufficiency within the
community. This role is pivotal in ensuring that families receive
the resources and support necessary to thrive. The ideal candidate
has a passion and talent for helping others, communicating clearly
and compassionately, and has the ability to connect and empathize
with individuals, including diverse and marginalized populations.
They are detail-oriented, efficient, and highly organized. They
remain calm in high-stress situations and are able to work without
direct supervision. They are trustworthy, dependable, relatable,
and friendly. The ideal candidate is familiar with social services
in the Missoula community, has knowledge of mental illness and
treatment, has two or more years of experience providing case
management or similar services, and has a bachelors degree in
social work or human services or a related field. Who We Are

supportive housing, mental health care, employment and education
support, trauma-informed childcare, and more. We work closely with
young families, donors, and community partners to build brighter
futures, two generations at a time. What Youll Do The Adult Case
Manager is responsible for the following: Providing direct support
to clients, including assistance with paperwork, goal planning, and
skill-building. Maintaining accurate and timely client
documentation, including dates, times, and progress notes for all
meetings, in the electronic health record system. Building rapport
with clients while maintaining professional boundaries at all times
Providing services primarily in the community or clients homes
Providing staff with client history/special requirements and
hosting team meetings Making referrals to other services such as
mental health services, parenting, etc. Communicating with clients
therapists for care coordination and treatment planning Providing
transportation and serving as an advocate for clients at
appointments Completing and overseeing the discharge process for
all assigned clients Meeting and maintaining billing hours
expectations and documentation timelines Attending meetings and
training as required and meeting regularly with their supervisor
Other appropriate and job-related tasks as assigned Requirements
(Education, Experience, Knowledge, Skills, and Abilities):
Bachelors degree in social work, psychology, rehabilitation
counseling, education, vocational rehabilitation, or a similar
field and/or related experience At least 2-3 years of experience in
supported employment, vocational rehabilitation, mental health
services, or related fields. Experience working with individuals
with mental health conditions, disabilities, or other barriers to
employment is strongly preferred. Familiarity with the Individual
Placement and Support (IPS) model or similar evidence-based
employment programs is preferred. Certification in Supported
Employment (CESP) or related credentials are a plus CPR/First Aid
certification (or willingness to obtain it within a designated time
frame) is preferred. Any state-specific licensure or certification
(e.g., Licensed Professional Counselor, Licensed Social Worker) is
preferred but not required, depending on program needs. Familiar
with social services in the Missoula community Experience providing
services to young adults with special needs, severe emotional
disturbances, and/or cognitive or developmental delays. Who You Are

You Should Know This position is full-time, 32 hours/week. The
schedule for this position is M - F (9 a.m. - 5 p.m. with the
expectation of occasional evening shifts no later than 8 p.m. as
dictated by client need. Possibility for 4 - 10 hour shifts. Our
goal is to meet the needs of the families served as well as the
hired individual. This position reports to the Lead Case Manager
and does not supervise others. The role requires local travel (~
30% of the time) in the community, client homes, or office
utilizing company vehicles. The hourly wage range for this position
is Level 2 ($19.00 - $26.00/hr) with the hired individuals wage
calculated using Mountain Homes equitable wage matrix. Available
